「活夠久」不想洗腎 SDM讓他決心接受治療

一名85歲老伯伯，10多年前因腎腫瘤，切除左腎， 8年前因右腎功能開始異常，由腎臟病個案管理師收案穩定控制多年，直至今年4月腎功能急速惡化，恐面臨洗腎，老伯伯認為自己年事已高「活夠久了」，原想放棄治療進入安寧，經個管師評估病人癌症並未復發轉移，也沒有其他慢性病合併症，身體還算硬朗，生活也能自理，且不符合安寧收案的資格等狀況，多次跟病人充分說明有關末期腎衰竭治療模式，經病人與家屬共同討論後決定接受血液透析，已做好動靜脈瘻管，須要時可開始血液透析。

不放棄治療 SDM讓病人參與醫療決策

國泰綜合醫院腎臟內科主任彭聖曾指出，為響應衛生福利部委託醫策會推動的「醫病共享決策(Shared Decision Making, SDM)」，讓病人能參與自己的照護決策，自去年4月起針對末期腎臟病變進行SDM，透過加強圖文設計的衛教單張、影片等，跟病人充分說明，統計發現透析瘻管預先準備率從2017年1-5月的44%提升為6-11月的85.8%。而除了慢性腎臟病，目前已進行SDM的疾病包括糖尿病、冠狀動脈心臟病、腦中風、乳癌、小兒/成人中耳炎等。

醫病共享決策宣導活動 盼提升健康識能

為推廣SDM觀念，國泰綜合醫院特別舉辦病人安全週，以「醫病共享決策。醫病溝通創意作法」為主題進行系列宣導活動，不僅設置「病人安全我會『應』」響應專區，同時辦理「護『腎』逗陣來-醫病共享決策」主題講座，由腎臟照護團隊腎臟內科彭聖曾主任、衛教師、藥師、營養師等，針對腎臟病的保健、用藥及營養的照護日常與民眾溝通釋疑；另為宣導民眾正確就醫觀念，辦理醫病共享決策闖關活動也擴大讓民眾共同參與，藉由活潑生動的互動過程，提升醫療照護的健康識能。

轉職潮消退 3月失業率18年新低

【台灣醒報記者楊蕓台北報導】「3月失業率為3.66%，較2月下降0.04個百分點，也較去年同月下降0.12個百分點，創下18年來同月的最低紀錄。」行政院主計總處23日發布「3月份失業率統計」，國勢普查處副處長潘寧馨分析，由於春節年後轉職者已陸續找到工作，導致3月失業率偏低，也象徵就業市場穩定改善中。

主計總處報告指出，3月就業人數為1140萬3千人，較上月增加6千人；失業人數為43萬3千人，較2月份減少5千人。其中初次尋職失業者減少2千人；因工作場所業務緊縮或歇業，以及季節性或臨時性工作結束而失業者都分別減少2千人；而對原有工作不滿意而失業者則微增1千人。

「與去年同月相比，失業人數減少1萬2千人；1至3月失業人數平均為43萬3千人，也較去年同期減少1萬5千人。」潘寧馨表示，3月失業率為3.66%，較2月下降0.04個百分點，創下近18年同月新低。他分析，「3月份失業率下降，主要是農曆春節後轉職者陸續找到工作，也表示目前就業市場趨於穩定，勞動市場也穩定改善中。」

不過，潘寧馨指出，青年失業率仍偏高。按年齡層觀察，3月份失業率以20至24歲者11.75%最高，但較2月下降0.14個百分點，「主因是多為初次尋職，且處於職場調適階段，導致失業率較高」，15至19歲者失業率為8.06%，25至29歲者的失業率則降為6.4%，下降0.06個百分點；按教育程度別觀察，以大學程度者失業率5.05%最高，高中職及研究所則分別為3.60%及2.87%。
